Help Her Build a Future Beyond Foster Care

For many young women, turning 18 represents freedom, opportunity, and the beginning of adulthood. But for a young woman aging out of foster care, that same milestone can mean stepping into adulthood without stable housing, consistent relationships, financial security, or the family safety net many of us take for granted.

Healthy H.E.A.R.T.S. is working to change what comes next.

We believe a young woman should not have to choose between simply surviving and building a future.

Through our A.C.T. (Addressing Critical Transitions) Transitional Housing Program, Healthy H.E.A.R.T.S. provides young women ages 18–21 aging out of foster care with A Safe Place to Land while they prepare for lasting independence.

Our work is about much more than housing.

We know that programs alone do not transform lives. People do. Relationships do. Consistency does.

That is why our A.C.T. Transitional Home is intentionally designed to feel like a home, not an institution. It is a safe, nurturing environment where each young woman is known as an individual, surrounded by people who believe in her potential and are willing to walk beside her as she determines what comes next.

Residents receive individualized support and opportunities to:

• Pursue education, training, and sustainable employment
• Build financial literacy and independent-living skills
• Learn practical everyday life skills
• Access physical and emotional wellness resources
• Develop healthy relationships and stronger support systems
• Receive mentorship, guidance, accountability, and encouragement
• Set personal goals and create a plan for life beyond the program
• Build the confidence to make informed decisions and advocate for themselves

Our goal is not simply to provide a young woman with a place to stay. Our goal is to help her build a foundation strong enough to stand on when she leaves.

It Takes a Community

We believe no single organization can do this work alone.

Healthy H.E.A.R.T.S. works alongside child welfare professionals, courts, faith communities, nonprofit organizations, businesses, volunteers, mentors, and community leaders to help close the gaps young people can encounter as they transition from foster care into adulthood.

In 2026, we were honored to partner with Judge Delia Gonzales, the Dallas County Child Protection and Permanency Court, Harvest Family Life Ministries, and other community organizations for Project Graduation.

Together, we celebrated young people who achieved an important educational milestone while navigating the child welfare system. More importantly, we had the opportunity to remind them:
