You Can Be a Part of the Solution
Health to Home is a collaborative program between Texas Health Presbyterian Hospital Dallas and Austin Street Center (ASC). Since its inception in late 2020, the program has enrolled and served 427 unhoused individuals. This program specifically serves individuals experiencing homelessness who also have acute medical and behavioral health needs. Without access to insurance or affordable health care, emergency rooms are often the only recourse for this population, and once discharged they rarely have the support and space to recover.
Services:
Through strategic partnerships medical recuperation services are offered to adults experiencing homelessness using individualized holistic medical, behavioral health and wraparound social services that address the underlying causes of homelessness. Medical support is provided with physician and nurse oversight to eligible individuals for an average of 4 to 6 weeks depending on the complexity of the individual’s needs.

27-bed unit housed inside Austin Street Center, a homeless shelter, in Dallas.
