In Allen, Texas, a new chapter of hope and healing has begun. The Community Resource Center provides Collin County residents with access to condition specific services such as bloodwork, personalized education and care tailored to chronic health conditions, including diabetes, high cholesterol, high blood pressure and congestive heart failure.
The Community Resource Center is a multi-faceted intervention that improves health outcomes, fosters community connectivity, and empowers and educates Collin County residents.
The Texas Health’s 2022-2025 Community Health Needs Assessment (CHNA) determines areas in North Texas that struggle with non-medical drivers of health. Overall, 50% of health influences are traced to Zip Codes.
In 2023, Texas Health served 4,167 patients on Medicaid or without health insurance from the five Zip Codes surrounding Texas Health Allen.
With the Community Resource Center focusing on high-risk populations, a primary funding need for this program is to have the costs of patient care covered.
In June 2026 we have had a total of 631 referrals and 179 patients enrolled in the program. High blood pressure is still our #1 diagnosis followed by diabetes. The Community Resources Center has had 18 patients graduate, whose A1C improved by 23% and BP improved by 8%. Our hours are currently extended on Wednesday to assist with patient’s schedules. We have distributed 9,602 pounds of food with our biggest collaborators are ACCC and AHA. The total cost savings, avoiding the Emergency Department and inpatient visits, has been $5.3 million since the start of the program.
Through this project, Texas Health strives to enhance access to healthcare for individuals and families living in high-needs areas within Collin County. In addition to meeting immediate needs, we aim to educate, empower, and connect patients to community-based resources that will help them better manage their health throughout their lives.
Join us in making a difference by supporting this vital initiative. Your contribution can help ensure that everyone in our community has access to the care they need.
