For many patients living with chronic conditions, leaving the hospital is when a new set of challenges begin.
How will they get to their follow-up appointment? Can they afford their medication? Do they have the equipment they need to monitor their condition? What happens when their symptoms change and they aren't sure what to do?
The JPS Chronic Disease Management Program helps bridge those gaps and thanks to a generous matching gift from Ramel Company, the first $10,000 raised will be matched dollar-for-dollar, doubling the impact of every donation.
Every year, the JPS Chronic Disease Management Program supports approximately 1,500 patients living with chronic conditions such as heart failure, diabetes, hypertension, chronic kidney disease, and COPD. For 60 to 90 days after discharge, patients can receive personalized nurse case management, remote patient monitoring, education, and help accessing the resources they need to continue managing their health.
For some patients, that means a blood pressure monitor. For others, it may mean transportation to a follow-up appointment, help with medication costs, or other essential resources.
Recovery doesn’t end at discharge. Neither should our support.
