For many patients living with chronic conditions, leaving the hospital is when a new set of challenges begin.
How will they get to their follow-up appointment? Can they afford their medication? Do they have the equipment they need to monitor their condition? What happens when their symptoms change and they aren't sure what to do?
The JPS Chronic Disease Management Program helps bridge those gaps and thanks to a generous matching gift from Ramel Company, the first $10,000 raised will be matched dollar-for-dollar, doubling the impact of every donation.
Every year, the JPS Chronic Disease Management Program supports approximately 1,500 patients living with chronic conditions such as heart failure, diabetes, hypertension, chronic kidney disease, and COPD. For 60 to 90 days after discharge, patients can receive personalized nurse case management, remote patient monitoring, education, and help accessing the resources they need to continue managing their health.
For some patients, that means a blood pressure monitor. For others, it may mean transportation to a follow-up appointment, help with medication costs, or other essential resources.
Recovery doesn’t end at discharge. Neither should our support.
The JPS Foundation is a non-profit organization dedicated to raising money to provide goods and services to continue to make JPS Health Network a regional and national leader in transforming healthcare delivery. Our three pillars of giving are our patients, our community, and our team members.
As the county’s essential hospital, JPS provides healthcare to all Tarrant County residents, regardless of their ability to pay. While government funding plays a role in helping us fulfill that commitment, the JPS Foundation raises funds to fill the gap between that funding and the additional resources needed to provide the high-quality care our patients deserve.
There are funding needs at every donation level and each one makes a HUGE difference in the lives of our patients.
$35 – Provides transportation to an important follow-up appointment.
$50 – Provides a blood pressure monitor or pulse oximeter to help a patient manage a chronic condition at home.
$100 – Provides clothing and other discharge essentials for a patient in need.
$250 - Helps provide diabetes monitoring supplies, medication, or durable medical equipment for patients recovering at home.
$500 - Helps multiple patients receive transportation, medication, and home recovery resources.
$1,000 – Helps furnish life-changing equipment like wheelchairs, hospital beds, and supports multiple patients with essential recovery needs.
