Years without consistent healthcare had taken their toll. Jenn was living with uncontrolled diabetes, dangerously high blood pressure, chronic pain and multiple health conditions that made everyday life difficult. Her A1C had reached 11.7, and at nearly 500 pounds, even walking had become a challenge.
"I was literally dying," Jenn remembers.

Working alongside her HSNT care team, Jenn brought her diabetes under control. Her A1C, once 11.7, now consistently stays around 6.1. She receives ongoing care for her other complex health conditions and has incorporated behavioral healthcare into her journey.
Jenn began traveling with friends. She went camping. She returned to work for a time. Most importantly, she stopped being afraid of healthcare.
Today, Jenn has another number in mind: 70.
The women in her family who experienced similar health challenges didn't live to see 60. Jenn is determined to change that story.
"That's not my goal. I want to see 70."
