One patient, one team, coordinated care, whether you're 5 or 85, insured or not.
What your gift does
$50 - Funds one pediatric well-check for a child without insurance
$100 - Covers one mental health counseling session
$250 - Supports one month of chronic disease monitoring (BP/glucose checks) for one patient
$500 - Provides one mobile wound care visit for a homebound patient
$1,000 - Funds one diagnostic imaging or laboratory workup for an uninsured patient
$2,500 - Covers a month of coordinated primary, pediatric and mental health care for 5 uninsured patients
