Community Kidney Care is a 501 (c)3 nonprofit organization founded in 2017, organized to provide support to individuals with chronic health conditions by offering preventative health services, health education support groups, disease prevention,, and disease management programming. Direct care services are provided to individuals and families affected by health conditions. Programming includes, but is not limited to disease support groups, health coaching, physical fitness education classes, nutrition education classes, and more.
Support for community medical screenings to combat the rise in diabetes and hypertension in North Texans (Which leads to Chronic Kidney Disease). Financial support and program volunteers for CKD patient summit events, symposiums, small group education, small group trainings. Assistance is needed to provide financial support to patients that do not have the means to get to doctor appointments, dialysis, labortory appointments., etc.
Community Kidney Care developed a mission to cultivate healthier communities by advocating, mentoring, and promoting early detection of disease in order to assist community members in becoming more knowledgeable about the prevention and management of various diseases that lead to Chronic Kidney disease. Our organization helps to improve overall well being, with an emphasis on educating community members about the importance of lifestyle management through a holistic approach. Participants learn to incorporate holistic ways to adopt lifestyle changes for improved health by actively engaging in Community Kidney Care's disease prevention health services and participating in various community health initiatives.
