The mission of the Heart to Heart Hospice Foundation is to improve the quality of life for those with life-limiting conditions and those nearing the end of life. We carry out this mission by providing education to healthcare professionals and community members, and by providing support to those with critical unmet needs, including Veterans, patients suffering from Alzheimer's and dementia or cardiovascular-related illnesses, in addition to those living with financial hardship.
At Heart to Heart Hospice Foundation we believe it is our responsibility to promote and provide quality of life for those with life-limiting conditions, and the family members who are caring for them. We carry out this mission by providing supportive programs to those with the greatest un-met need in the communities that we work in, including Veterans, patients suffering from forms of dementia, cancer and cardiovascular-related illnesses, and those living with financial hardship. We also provide educational opportunities and scholarships to healthcare providers and to community members, because we believe that this increases the impact of our mission. Through the creation of the Heart to Heart Hospice Foundation, and our corporate dedication to its mission, we have dedicated ourselves to improving life for our patients, families and communities.
