Since 2004, the Community Health Clinic (CHC) has acted as a centralized healthcare resource for underserved patients, allowing them to avoid being over-referred to multiple doctors. Coordinated care plans also work to prevent or reduce the necessity for repeated emergency room visits, which present burdensome costs both to patients and to local hospital systems. Access to safety-net healthcare, such as what CHC provides, therefore not only benefits CHC's patients, but also helps to reduce hospital costs associated with non-urgent emergency room and in-patient care.

CHC has demonstrated a track record in helping patients manage chronic conditions through a primary care medical home. In 2020, CHC had 3,098 patient visits (941 unduplicated patients) consisting of primary care, physical therapy, dietitian and pediatric appointments. Over half (58%) of diabetic patients receiving care at CHC improved their Ha1C levels over a two-year period, and just under half (48%) of patients with hypertension improved blood pressure readings over the same time period. These improved biometrics are indicative of better chronic disease management and translate to fewer complications requiring intensive and costly care.

Ultimately, CHC delivers a much-needed resource for medically underserved Collin County families, providing an essential public service to the health and welfare of the community in treating chronic conditions and promoting healthy living to ensure patients are able to participate fully in their work and home lives. CHC provides healthcare at no cost to these patients-care that also includes needed lab work, imaging services, and a prescription-assistance program. This benefits not just patients, but the greater Collin County community: a healthy community better prevents the spread of diseases and reduces the number of people untreated and uneducated about their health.

CHC is primarily an all-volunteer clinic, including five volunteer physicians of varying specialties, fifteen volunteer nurse practitioners, and a volunteer physical therapist. All CHC nurses are volunteers, and when including front office and interpreters, CHC totals over sixty volunteers powering its mission to provide care to the medically underserved.

Our Needs

At the Community Health Clinic we have been impacted by COVID-19 as is everyone. As this virus progresses, financial support is needed now more than ever. We have already seen an increase in calls for healthcare as many families lose their jobs and their insurance. The Community Health Clinic will be there for those families to meet their healthcare needs until they are able to gain employment and insurance again.

The Community Health Clinic recently moved to a larger facility increasing our capacity to serve the community. CHC is one of the longest established charitable clinics in McKinney and has been a leader in the community providing free and charitable healthcare to thousands of families since 2004.

Donations will be used for ongoing medical care of our patients and future patients and to assist us in our growth.
